What You Will Need On Your First Visit
Health History Form
It is very helpful if we have certain information available at your first visit. If possible, often we will have mailed, faxed, or emailed a copy of our health history form for you to have filled out prior to your arrival. This information is important for us in order to be aware of any factors that may affect the treatment choices that are available to you.
Vital information includes any past or present illnesses, conditions, allergies, concerns as well as a detailed list of current prescription and over the counter medications that you are taking. Often, patients who take several medications find it is easier to just bring the bottles themselves in a bag.
Dental Insurance Card (If applicable)
If you have dental insurance, be sure to bring your dental insurance card with you. Please be aware that this is usually a different card than your medical insurance card.
Current X-Rays
If you have dental x-ray records that are less than two years old, try to pick them up or have them sent to us to arrive before your appointment. See our “Consent for Medical Records Release” form if you need it. Be aware that some other offices can be slow to mail us x-ray film records.
If your records are more than two years old or are unavailable, we will be glad to make current images at your appointment.
